Application Requirements
You can Select either one option to submit your application.

Basic Requirements
-Minimum age of 21
-Minimum annual income of RM18,000.00

DOCUMENT REQUIRED
Option 1 / Pilihan 1 
For existing Credit Cards / Charge card holders more than 6 months (Bagi kad kredit yang
sedia ada yang melebihi 6 bulan)
-Photocopy of I.C. (both sides).
-Photocopy of current credit card (both sides).

Option 2/ Pilihan 2
For MNC, Public listed company, Government employee (Syarikat senarai di Bursa Saham dan 
kakitangan kerajaan)
-Photocopy of I.C. (both sides).
-Minimum basic salary of RM1200 + fixed allowance.
-Latest two (2) months salary slips.
-If basic salary below RM1500, EA form with RM18,000 per year and above must be attached.

Option 3/ Pilihan 3
For small and medium size company's employee (Perkerja di Sendirian Berhad atau enterprise)
-Photocopy of I.C. (both sides).
-Minimum basic salary + allowance + comission of RM1500.00 per month 
-Latest two (2) months salary slips 
-Please support with either one of the documents :(1) Latest Bank Statement
                                                             or
                                                  (2) EPF statement
                                                             or
                                                  (3) Return TAX J Form

Option 4/ Pilihan 4
For fresh graduate (Pelajar yang baru tamat di instituisi tinggi sama ada di kolej dan 
universiti
-Photocopy of I.C. (both sides).
-Minimum basic salary + allowance + comission of RM1500.00 per month 
-Diploma, degree, Master etc. photocopy of your certificate
-Latest two (2) months salary slips or / atau Empoyment letter.

Option 5/ Pilihan 5
Self-employed / sole proprietor ( Perkejaan yang diusahakan sendiri)
-Photocopy of I.C. (both sides).
-Minimum 1 year in business  
-Latest Form J (with minimum RM24,000 income).
-Latest 3 months' bank statements.  
-Business Registration Certificate or Form 9/24/49 or Form A, D. 

Option 6/ Plihan 6
Commission Earner (Pendapatan Perkerja melalui komisen)
Included insurance, real estate, unit trust, MLM, motorcar sales agent.  
-Photocopy of I.C. (both sides).
-Minimum 1 year in business.
-Latest 2 year Form J (with minimum RM24,000 p.a. income).
-Only EA Form from MNC & PLC will be accepted. 
-Latest 3 months commission statements. 
-Latest 3 months bank statement (commission bank-in account). 

Option 7/ Plihan 7
Bankruptcy or those are unable to proove the income 
(Bankrupsi atau sesiapa yang tidak berupaya membuktikan pendapatan)
-Photocopy of I.C. (both sides).
-Fixed deposite of Min. RM 5,000.00 with AmFinance
